Obv: A stylized projection of the Medical Center being erected, with panorama of Jerusalem in the background. The inscription on the rim reads: "In the service of Jerusalem". The English translation appears on the lower part of the medal. Rev: Above, to the right, the emblem of the Sha'are Zedek Hospital in Jerusalem. In the centre: "Shaare Zedek Centenary" and the translation in English. The dates: "5633-5733" and "1873-1973". Mintage: 3100 pcs. Number on the edge: 2362. Designed by Yaacov Enyedi.
